The Department Agriculture Central Office – ICTS-NOMD, through its Bids and Awards Committee, intends to engage services of an ICT Consultancy Services, ISO through Highly Technical Consultants under Section 53.7 of the 2016 Revised Implementing Rules and Regulations of Republic Act No. 9184.

The Terms of Reference (TOR) for the Technical Guidance/Consultancy Services in the Development and Implementation of Information Security Management System (ISMS) Certifiable to ISO/IEC 27001:2013 for the Information and Communications Technology Service (ICTS) is attached for your guidance/reference.
Qualified suppliers are invited to submit their quotations/proposals duly signed by you or your duly authorized representative not later than November 14, 2019 at 12:00 noon subject to the Terms and Conditions provided at the last page of this RFQ.
A copy of your (1) 2019 Mayor’s/Business Permit, (2) Professional License/Curriculum Vitae and (3) PhilGEPS Registration Number are required to be submitted along with your accomplished quotation.
Only sealed quotations shall be considered by the BAC. Bidders/Suppliers shall reflect on the envelope the RFQ Number with Company Name. Quotations should be submitted at the Office of the Procurement Division, 2nd Floor, ICTS Building, Department of Agriculture – Central Office, Elliptical Road, Diliman, Quezon City.

TERMS AND CONDITIONS
- Bidders shall provide correct and accurate information required in this form.
- Price quotation/s must be valid for a period of One Hundred Twenty (120) calendar days from the date of submission.
- Price quotation/s, to be denominated in Philippine peso shall include all taxes, duties, and/or levies payable.
- Quotations exceeding the Approved Budget for the Contract shall be rejected.
- Award of contract shall be made to the lowest quotation which complies with the technical specifications, and other terms and conditions stated herein.
- Any interlineations, erasures, or overwriting shall be valid only if they are signed or initialed by you or any of your duly authorized representative/s.
- The item/s shall be delivered according to the requirements specified in the Technical Specifications.
- In case of two or more bidders are determined to have submitted the Lowest Calculated Quotation/Lowest Calculated and Responsive Quotation, the BAC/TWG shall adopt and employ “draw lots” as the tie-breaking method to determine the single winning provider in accordance with GPPB Circular 06-2005.
- The BAC/TWG/End-user shall have the right to inspect and/or to test the goods to confirm their conformity to the technical specifications.
- Liquidated damages equivalent to one tenth (1/10) of one percent (1%) of the value of the goods not delivered within the prescribed delivery period shall be imposed per day of delay. The HOPE shall rescind the contract once the cumulative amount of liquidated damages reaches ten percent (10%) of the amount of the contract, without prejudice to other courses of action and remedies open to it.
- The Uniform Guidelines for Blacklisting set forth in Appendix 17 of the 2016 Revised IRR of RA 9184 shall apply in case any of the violations listed under Items 4.1 and 4.2 thereof is/are committed without prejudice for forfeiture of performance security.
- The Department of Agriculture reserves the right to reject any and all bids, declare a failure of bidding, or not award the contract at any time prior to contract award in accordance with Section 41 of RA 9184 and its IRR, without thereby incurring any liability to the affected bidder or bidders.
